Types of adult transitions we support

We support a range of adult transitions, delivered proportionately and with appropriate risk management and professional oversight.

This may include:

  • Prison to hospital or medical appointments

  • Hospital to prison or secure settings

  • Transfers between custodial or secure environments

  • Court-related journeys

  • Other planned or urgent adult transitions where specialist support is required

Support is planned and delivered with careful consideration of individual risk, vulnerability and context. We prioritise clear communication, predictability and proportionate responses, recognising that transitions may involve heightened anxiety, distress or uncertainty.

All adult transition support operates within a clear safeguarding, risk management and governance framework. This includes defined professional boundaries, robust information handling, incident reporting, and oversight from senior leaders with extensive experience across custodial, healthcare and secure environments. Practice is guided by organisational policies and relevant legal and professional standards.

Professional Expertise and Service Boundaries

Excel Safe Transitions is led by senior professionals with long-standing experience supporting complex, high-risk and sensitive transitions across custodial, healthcare and secure settings. This ensures that adult transition support is delivered with appropriate judgement, oversight and accountability.

Adult transition services are commissioned and delivered separately from our children and young people’s services, with planning tailored to the specific needs, risks and circumstances of the individual. Services are developed in response to commissioning requirements and may evolve over time.
